As a local, non-profit company headquartered in Arizona, Blue Cross Blue Shield of Arizona (BCBSAZ) treats you like a neighbor. Plus, the Blue Cross and Blue Shield symbols are recognized throughout the United States and in many parts of the world. Nationwide, over 80 million people - one in four Americans - have put their trust in Blue Cross and Blue Shield Plans
to provide their health care coverage.
For Individuals Under Age 65:
Blue Cross Blue Shield of Arizona has health plans specifically designed for Arizona residents who don't have health insurance through their employer. Blue Cross Blue Shield of Arizonas individual health plans offer many of the same type of health benefits big companies offer to their employees. These plans are designed specifically for Arizona residents who dont have health
insurance through their employer.
One plan cant meet everyones needs. Thats why BCBSAZ created three different health plans for individuals to choose from - plans to suit their individual needs and budget:

BluePreferred®
is a preferred provider organization (PPO) plan with deductible choices ranging from $250 to $5,000. With this plan, you pay a modest copayment for visits to PPO doctors (without having to meet your deductible first). Your benefit level is greatest when you use providers in the PPO network, but you also have the flexibility of using providers outside this network. Plus, when
you travel you have access to a national network of PPO providers from independent Blue Cross and Blue Shield plans throughout the country.

BlueClassic®
is a traditional indemnity plan that gives you complete freedom of choice with regard to your hospital or physician; plus you can realize cost savings whenever you use a provider that participates with Blue Cross Blue Shield of Arizona or other independent Blue Cross and Blue Shield plans nationwide. There are six deductible options available, from $250 to $5,000.

BlueSelect®
is a health maintenance organization (HMO) plan that's ideal for those who don't want to bother with deductibles and coinsurance payments. You simply make fixed copayments when you receive most covered services. Unlike some HMO plans, BlueSelect does not require that you obtain a referral from a PCP for visits to network specialists.
For Groups:
No cookie-cutter approach to health plans here. Blue Cross Blue Shield of Arizona (BCBSAZ) offers employer groups flexible choices that include PPO, PPO Upgrade Options, HMO, Dual Option (PPO and HMO), Open Access HMO, and Dental plans - choices that can accommodate practically any employers budget. for employer groups has all the features and benefits of BluePreferred PPO plans, plus more choice. BCBSAZ PPO Upgrade Options provide employers the opportunity to allow their employees to be part of the health insurance purchasing decision.

for employer groups offers three distinct stand-alone plans with different office visit and hospital inpatient copayment options.

Dual Option Plans HMO/PPO
for employer groups is a dual option plan lets employees who want the 'no deductible, no coinsurance' simplicity of an HMO choose BlueSelect.

BlueAdvantage
(a BlueChoice® plan) for employer groups offers employees a healthy dose of flexibility and freedom of choice.

DentalChoice
is an indemnity plan with the freedom to choose any dentist, DentalChoice provides coverage for a wide range of dental services.
